To Start the Referral Process to the Outpatient Clinic:
Call CVFC Outpatient Clinic at (518) 561-8480. Walk-ins are accepted during Open Access hours. Assessments start with an Intake session to complete the required paperwork. A comprehensive Assessment is performed by one of our counselors. This Assessment may take 1 - 3 sessions. We ask that you bring: your insurance information, a list of current medications, and any relevant referral documents or contact information. Our goal is to engage you in the appropriate level of care as soon as possible.
To Start the Referral Process to the inpatient Recovery Campus:
Referring providers may download and print the Referral Form below. Fax our referral form and other relevant forms to (518) 643-2456. You may also call (518) 643-0148 to speak with the Program Director or the Clinical Supervisor.
New residents may check the “WHAT TO BRING INFORMATION” for a list of what items are/are not acceptable to bring to the facility.

This facility is open 24/7 and is walk-in friendly. No referrals needed. You may call the Crisis Center at (518) 324-4700 if you have questions about admission or services.
