Clinton County Jail Partnership

Champlain Valley Family Center partners with the Clinton County Jail to provide onsite substance use disorder (SUD) assessments and coordinated treatment referrals. CVFC staff meet directly with incarcerated individuals to assess their treatment needs, offer support, and help establish a plan for continuing care upon release.

CVFC also collaborates with the jail on a Vivitrol® Program, with the shared goal of supporting a safe and effective transition to community-based treatment that includes access to medication assisted treatment (MAT).

Services Provided

  • Onsite SUD assessments for individuals in custody
  • Linkage to care through coordinated treatment referrals and transition planning
  • Connection to medication assisted treatment, including the Vivitrol® Program
  • Support in accessing community treatment services immediately upon release
