Care Management

Champlain Valley Family Center offers comprehensive Care Management services for both adults and children through our partnership with AHI’s Health Home. This program supports Medicaid eligible individuals—including youth and families—who are experiencing chronic medical conditions, mental health challenges, or substance use disorders.

Our Care Managers work closely with clients, parents, and service providers to coordinate appointments, enhance communication between systems, and develop personalized care plans tailored to each person’s unique needs. For children, this may include collaboration with schools, pediatric providers, and family support systems to promote stability and healthy development.

The goal of Care Management is to improve overall health outcomes, reduce stress on individuals and families, and create lasting stability in daily life.
