Care Management

Champlain Valley Family Center’s Recovery Campus (CVFC-RC) provides Stabilization and Rehabilitation services in a Residential setting in Schuyler Falls, NY. This is an Office of Alcohol and Substance Abuse Services (OASAS) certified program. The facility has 16 beds to serve men and women, age 18 and older, who have an identified substance use disorder and are in need of a more intensive level of care. It is a 24 hour a day, 7 day a week program with full time medical and clinical staff supervision.  To make a referral to the CVFC Recovery Campus call (518) 643-0148. The program provides:

  • Medical Stabilization and Withdrawal Management
  • Medication Assisted Treatment
  • Individual and Group Counseling
  • Educational Lectures
  • Recreational Activities
  • Life Skills
  • Leisure Skill Activities
  • Yoga, Mindfulness, and Stress Management
  • Family Therapeutic Sessions
  • Vocational and Educational Counseling
  • Psychiatric Evaluation and Medication Management
  • Comprehensive Discharge Planning

These individuals can be referred from outpatient treatment programs, hospitals, legal entities, family members, themselves, or other community agencies.

Champlain Valley Family Center offers Care Management as part of AHI's Health Home.

A Health Home is a care management service that connects community and social supports with health care, and organization of medical and behavioral health care.  It is a partnership of primary care providers, hospitals, behavioral health organizations and community-based organizations.

Care Managers provide a single contact for healthcare and community service needs for a specific population (NYS Medicaid or Medicaid eligible recipients who meet certain criteria, such as having two chronic medical conditions) in order to improve care coordination. It is the hope that all of the caregivers involved with an individual communicate with each other with an eye toward a better focus on their mutual client's varied needs.
